We provide psychotherapy, psychiatric care, ketamine-assisted therapy, and psychedelic integration services. This policy applies to all interactions with our practice, including in-person care, telehealth services, and digital communications. We collect personal information such as name, date of birth, contact details, emergency contacts, and insurance or payment information, as well as protected health information including medical and psychiatric history, symptoms, diagnoses, treatment plans, medication records, session notes, and information related to ketamine or other treatments.
